Initial Evaluation

First-time evaluation of physical or occupational therapy needs

This Initial Evaluation session may be conducted in home or in clinic to determine the plan of care and necessary Ongoing Treatment sessions needed for your child.
Please allow 45-60 minutes for the therapist to conduct the evaluation.
Ongoing Treatment​
Follow-up sessions based on Treatment plan​
This appointment time is for one Ongoing Treatment session. Pricing varies by Individual Ongoing
Treatment Plan as determined at time of Initial Evaluation with your therapist.
Each session runs 45 or 60 minutes in length.
